Vital Statistics: Birth Certificates, Marriage Certificates, Death Certificates,
Marriage Licences


A legal change of name can only be processed in Alberta for residents of Alberta. Birth records have to be ordered from your birth jurisdiction after the legal name change has been approved. The applicant must be 18 years of age or older. All persons 18 years of age or older who are changing their name on the application require Finger Prints.
Depending on the circumstances, presentation of identification, court orders, consents, dispensations, statutory declarations, proof of marital status or custody orders may be required.
Once Vital Statistics approves and processes the legal change of name, a Change of Name Certificate is mailed to the customer. A new Birth Certificate can then be ordered.

The total fee of $180.00 for a Legal Change of name includes the Government Fee, a Finger Print handling fee and our fees. Additional fees are charged for Finger Prints and are available from either the RCMP or City Police.

A Legal Change of name is not required when:
Married Last Name
When a woman marries and wants to use her husband's last name, she may assume just his last name or her last name combined with his, the order of the two names is optional.
Correcting (Amending) Information on an Original Registration
In some cases, a change or correction to a person's name does not require a legal change of name.
Request to amend a given name
When a child is born, the parents complete a Registration of Birth recording the spelling of their child’s given name. Within the first 10 years of the child’s life the spelling of the given name has changed.
To amend the birth record, Vital Statistics requires the following:
Proof to support the correct given name. Supporting proof/evidence must show that the child used the new spelling of their name prior to their 10th birthday. Some examples of acceptable proof/evidence are a school record, an immunization card, baptismal/christening certificate.
Complete a statutory declaration

Note:
If the child is not yet 10 years old, a statutory declaration will be prepared, supporting proof/evidence is not required and a fee is applicable.
If the child is 10 years old or older and cannot provide proof/evidence, showing they used the new spelling of their name before their 10th birthday, an amendment cannot be processed and their only option is to proceed with the legal change of name procedure.
